This form is designed to help our media team develop projects more efficiently on a realistic timelines so that we can better serve our church congregation. Please fill out every question with as much detail as possible so that those creating on your behalf are well equipped.

*All media requests should be turned in to the Creative Project Manager at a minimum of 3 weeks prior to when needed. Special videos and event staffing request should be turned in at a minimum of 6 weeks prior to when needed.  You will be informed whether your request has been approved and if more information is needed by the Creative Project Manager within one week of submission. Once approved, your request will be given to the media staff person(s) responsible for completing your request.
